Health Care Innovation Award: Medical Neighborhoods Transform Rural Care Delivery

Wyoming’s health care challenges

Wyoming’s health care delivery system can be difficult to navigate for high-risk patients. The health care system is currently fragmented: Wyoming’s rural landscape makes it difficult for many to recieve the appropiate level of care, and communication between providers could be improved. This makes it difficult for high-risk patients to navigate the health care system, yet many other factors explain why seeking care in the emergency room may be the easiest option for many.
