Introducing the Mobile Integrated Health (MIH) Pilot Program – Enhancing Your Care Experience
We are excited to introduce you to the Mobile Integrated Health (MIH) pilot program, a pioneering initiative as part of Cheyenne Regional’s commitment to improving access to care and enhancing patient well-being within our community.
The MIH program integrates specialized paramedics and EMTs to deliver personalized care tailored to your needs in the comfort of your home and the community. This innovative approach is designed to provide comprehensive services for certain medical conditions, including COPD, CHF, hip and knee replacements and oncology care.
As a valued patient, you may be eligible to benefit from this program. If referred by your provider, you will have access to in-home care based on approved protocols, which include history and physical assessments, home medication review, post-discharge follow-up, home safety assessments and more.
Our goal with this program is to optimize your care experience, minimize hospital readmissions and ensure that your use of the Emergency Department aligns with your specific needs. We believe that this approach will not only enhance your recovery but also provide a higher level of convenience and comfort during your healing process.
Should you have any questions or would like to learn more about how you can benefit from the MIH program, please do not hesitate to reach out. We are here to support you every step of the way.
Discuss with your provider if this program would be right for you. If you have questions about the MIH program, please call (307) 633-7775
