Reasonable Alternatives Process

  1. Members can reach out to Member Services and request an Reasonable Alternative (RA), long term or short term RA. Forms are located in the support pages and can be sent in by email/fax.
  2. Short Term vs. Long Term
    • Short term: applicable for temporary medical conditions that last as long as it takes a member to recover. The form requires a window of time members expect to recover from their condition. Examples are pregnancy, surgery, chronic illness and injury.
      • A short-term Reasonable Alternative cannot have a start date more than 14 days prior to when the request is submitted. Client Delivery will need to approve of manual point adjustments for anything older than 14 days from the date of submission.
      • The end date must be filled in with a defined end date. It cannot be left open ended in any way ( N/A, TBD, completely blank, etc…).
    • Long term: applicable for medical conditions that members may not have a recovery time-frame for. The long-term form requires members to have a physician sign-off on their condition and indicate what activity and/or biometrics are feasible for the member to achieve. Examples are chronic health conditions and disability. The long term is the only RA that requires a physician signature.
  3. Form Submission and Approval/Rejection
    • Once the form is complete and sent in, Personify Health will review and then approve the form and send an email to the member with approval.
    • If it is rejected, an email will be sent stating the reason why.
  4. Reasonable Alternative Requirements
    • Members with Reasonable Alternative turned on will need to self-enter a Workout using the pre-populated list of exercises in order to earn the maximum 140 points for daily activity. They would need to do this activity everyday. However, they can login and backdate 14 days prior. It is not possible to backdate more than 14 days on the platform.
